Try a hard reset. Hold the home and power button at the same time. Ignore the slide to power off. Keep holding until you see the Apple logo. Wait 60 seconds ish and phone will be back to normal so should safari icon.

Are you jailbroken? If so that's been happening to bunch of people. Just do a reset or just respring your phone. Eventually it would go back to normal. It happen to me last night.

Mostly u are jailbroken. Such instances on non jailbroken are very rare.

U can go to sbsettings-system options- fix user dir permissions.

That will take care of all your white
Icons..

Yeah, I undid theme, then after 3 or 4 resprings it went back to normal. It was the camera app for me.

Also, open SBSettings>Dock application>Fix blanks. Thats the fastest way without installing anything else.
