I Googled it and found this:

"Long time later but I see more have asked. The message is related to a SIM Application that carriers purchase. Very dodgy, it vaguely refers to social address book , swaps out information among SIM and with network. I cancelled mine immediately. Can be found in Phone, SIM Applications."

So, what is the solution ? If this alert is triggered from the service provider then i have another iPhone with the same service provider but i have never ever received such alerts on other iPhone.

Is there a Settings > Phone > SIM Applications toggle?
If not, call your carrier.

You could try resetting all settings. If that doesn't work, you might want to restore to factory settings. Restoring would also likely help with the other problems you are currently experiencing with this iPhone.

The search for "sim alert" yields a number of possible reasons/solutions. One link suggested a non-official/standard SIM.
YMMV

Save yourself time/trouble and take the phone to your service provider and get a new sim.

This is your phone? It looks like a screenshot from a 3G/3GS. I thought you said you had a iPhone 5 and your wife has an iPhone 4?

Yes, you are right Europa. The screenshot is taken from my wife's iPhone4 and she is getting that alert.
