Is it asking for his voicemail password?

Yes. The phone keeps becoming deactivated. I'm trying a restore now

If it's the voicemail password, that is normal and you can ignore that. I got that plenty of times on my iPhone 4. What password is it asking for? Can you take a photo of it and post it here?

Deactivated? Well, that's not good. Hopefully a full restore will help. If it still continues, I would first contact your carrier.

It was asking for a password on the lock screen. I'm restoring it now and will report back.

I was able to restore and get past the lock screen but still had to call AT&T to reset the voice mail password that had NEVER been set in the first place!
