I had this time problem after making some changes to the DisplayOrder.plist file to remove/hide an icon or two (forgot to add a </dict>). I would check your DisplayOrder.plist file and make sure its formated correctly. You can find the file in the /System/Library/CoreServices/SpringBoard.app dir

I transferred a number from Nextel/Sprint. I was told at the end I would receive and e-mail. Now when I connect the phone iTunes gives me the same screen and I do not have the option to activate the iPhone.

I had the same problem....I started over, without using the pre approval credit code and I was successful. Now I am just waiting for the activation email....One hour and fifteen minuets and counting...
