If you need phone support after the first 90 days, you will need AppleCare. If not, I guess you can wait. But it doesn't really matter. It only covers 2 years after you buy the phone, not 2 years from when you get the AppleCare.

You can't. The only thing you can do is wait for it to arrive, then return it without opening it.

I'm pretty sure the first charge was a pre-approval charge that will go away and be replaced by the 2nd charge. The 2nd charge, I believe, is the actual charge they told everyone they would get when their phone actually shipped. That's just how charges are handled when you have a pre-approval...
