Missing phone icon in iPhone 4

Hi, can anyone please help me my phone icon in my iPhone has disappear. I have search for it but i can't find it anywhere.
I have search the phone app folder in my iPhone with iPhoneBrowser but i can't find it. Can anyone please help how to get my phone icon back and where to locate the phone app folder in my iPhone. I have an iPhone 4 5.01

Did you search for it in "spotlight" ? When you swipe all the way left and all you have is a search bar? Type in phone.

Try doing a hard reset/ reboot. Press both home & sleep buttons at same time... Ignore the slide to power off, keep holding until you see the Apple logo. See if that helps fix things.
