Read about it here. With the new version (Version 0.99.07), the sound will now work without having to disable mediaserverd in Terminal. You DO have to edit one file, but you only have to do it once.
1. Navigate to /System/Library/LaunchDaemons/coreaudiod.plist using Manzana, iBrickr, WinSCP, or the like. Find the part that looks like this:
<key>ProgramArguments</key>
<array>
<string>/usr/sbin/mediaserverd</string>
</array>
<key>OnDemand</key>
<false/>
Now just change that last <false/> to <true/>, reboot, and you're done! Sound will now work automatically every time you play, and you can still receive calls with sound while playing.
If you don't know how to edit files, the programmar already created an edited file for you. You can just download the package via the link above and look for the file coreaudiod.plist. Now just put it in System/Library/LaunchDaemons/ and overwrite the old coreaudiod.plist.

EDIT: These instructions used to say </true> instead of <true/> which is why people had problems. They should be correct now.
ANOTHER IMPORTANT EDIT: This guy does his updates way too quick! Now he has version "1.00_RC1" which does everything I just said automatically! Good riddens.
